Step 1:

Step 2:

Enclose all the following in an envelope:

a. The original completed Money Back Guarantee Form.

b. An unmodified store register receipt with any Depend® product purchased (limit one) between 1/1/25 – 12/31/25. Please circle product purchased and date on store receipt.

c. The original UPC Code from the package. UPC Code must match offer packaging.

Step 3:
